How to sign the release form

You may want to print this page to help you through signing the release form.

Adults (every person must be on their own line regardless of age)

• Read the release form
• Under SIGNATURES OF PERSONS EXECUTING THIS RELEASE have each adult sign their own names
• Have each adult check there riding ability under CHECK LEVEL OF RIDING ABILITY (if you don’t know your riding ability it’s probably poor, if your good your good and if your comfortable with some limited experience check fair.
• Have each adult initial (with their own initials) under PROTECTIVE HEAD GEAR REFUSAL. We understand and we encourage you to request and use a helmet when you arrive but we still need your initials here so that you can take off the helmet if you need to during the ride or event.
• Have each adult initial (with their own initials) under DECLARATION OF FITNESS TO RIDE
• Leave the line blank under IF RIDER IS A MINOR, PRINT AND SIGN NAME OF RESPONSIBLE ADULT

MINORS (EVERY PERSON MUST BE ON THEIR OWN LINE REGARDLESS OF AGE)

• Have the responsible adult read the release form
• Under SIGNATURES OF PERSONS EXECUTING THIS RELEASE have the responsible adult put the minor’s name.
• Have the adult check the riding ability of the minor under CHECK LEVEL OF RIDING ABILITY (if you don’t know the minor’s riding ability it’s probably poor, if it’s good it’s good and if they are comfortable with some limited experience check fair.
• Have the adult initial (with the adult’s initials) under PROTECTIVE HEAD GEAR REFUSAL. We understand and we encourage you to request and use a helmet when you arrive but we still need your initials here so that you can take off the helmet if you need to during the ride or event.
• Have the adult initial (with the adults initials) under DECLARATION OF FITNESS TO RIDE
• Have the responsible adult sign under IF RIDER IS A MINOR, PRINT AND SIGN NAME OF RESPONSIBLE ADULT. (make sure it’s on the same line as the minor’s info)
